Ivan Marquez, Death – An Ogden man was killed in an industrial accident in Clearfield, the second in the city since last May. The Clearfield Police Department identified the man on Thursday as Ivan Marquez, 27. According to a police department statement, the accident was reported early Wednesday morning after a piece of equipment fell on Marquez at Utility Trailer at 1111 S. 1000 West, adjacent to the Freeport Center.
“Though still being investigated, it appears a piece of equipment which was suspended above the workstation fell on the victim,” reads the statement. The Utah Occupational Safety and Health Division, part of the Utah Labor Commission, is investigating.
“When UOSH investigates an incident, they are not investigating the injury or fatality itself. We are looking for any violations of safety standards that may have led to the incident,” said Utah Labor Commission spokesperson Eric Olsen. The death of Marquez comes after William Davis, a 38-year-old Sunset man, died in a May 15 accident at Americold at 755 E. 1700 South. Olsen said the investigation into that incident is still open and he provided no additional information.
Davis somehow became pinned between a forklift and freezer shelves, leading to his death, officials said at the time. After Wednesday’s accident, North Davis Fire District and Clearfield Police Department attempted resuscitation efforts on Marquez before he was brought to Holy Cross Hospital in Layton in “extremely critical condition.” He had suffered head injuries and died a short time after arrival at the hospital.
UOSH reports 17 workplace deaths so far for the fiscal year that started Oct. 1 last year. In 2021, the last full year for which stats are available, 52 people died in workplace deaths in Utah, up from 48 in 2020, according to the Utah Labor Commission.
